Formative Assessment and I Can Tracker

This is a tool that I use frequently to keep formative data on my students and the standards that have been taught. This template can be altered electronically or you can print and complete it as you progress throughout the year. Student names should be added at the top and I Can statements derived from the standards should be added in the second column. As you use formative assessment to analyze student achievement use the tracker to record the results. This can be done in many ways. You can place a check in the box for students who get the answer correct. (I place Y for correct and N for incorrect, and I record which ever one is less to save time) Then place the percentage in the last column. Example is provided. Once this information is tracked you can decide which standards need to be re-taught based on the data or you can use it to group students strategically for differentiated instruction. The point of this tool is to track student progress, standards that have been taught, and then use this information to make data driven instructional decisions in the classroom.
