I got the idea for these from another TPT user, however I have fine-tuned them quite a bit. I currently have students on the autism spectrum that are unable to distinguish/answer simple WH questions. So I use these not only for IEP data but as practice and learning tools. I read the sentence to the student, have them read them back, and then ask the following WH questions. I always write their immediate responses with no prompting. Then after each question is answered we go back and use color

There are two things that are imperative to student's understand of reading. Those are the two that we don't spend the time to do adequately. The first of these is to have students make text-to-self connections (activating background knowledge). I have used this activity over and over with students.
